Office Policy

At the initial appointment, it is the policy of this
office to explain in detail each procedure to be performed
for each patient.

If appointments are not kept, we cannot be responsible
for successful endodontic treatment.

The fee for your treatment will also be presented at
your first visit and IS DUE IN FULL upon completion, unless
specific insurances are involved. A statement will be given
on the first visit for your benefit.

We will be happy to complete any insurance forms.
However, you are responsible for payment, unless specified
by the insurance company.

If it is necessary to make special arrangements, it must
be done with approval of the Office Manager.

When appointments are broken, full remittance is
expected before completion of treatment.

If at anytime, you feel dissatisfied with your treatment
in any way, please do not hesitate to inform me.
